Each September we are excited to meet new families, and also to welcome back our returning families.    Along with signing an enrollment contract, parents are required to submit the following forms for their child.

  • A Virginia Student Health Form must be on file at the Day School before your child begins school.  When your child receives subsequent immunizations, please provide us an update.

  • An Emergency Information Form ensures that we have contact information for parents, babysitters, and anyone who might be authorized to pick up a child.  We also want to have important medical information to keep your child safe.

  • A Child Profile Questionnaire helps us to know your child better.

We ask that all forms be submitted to the school by August 1.
